1. Application Form
2. PSA Birth Certificate (and PSA Marriage Certificate for married female applicants only)
3. Original Transcript of Records of **all** degrees earned (must have at least completed a 4-year bachelor’s degree)
4. Certified True Copy of Diploma or Certificate of Graduation of **all** degrees earned
5. Certificate of Employment accomplished on the organization’s official stationery/letterhead, signed by the applicant’s immediate supervisor (if applicant is employed)
6. Work Duties and Responsibilities of applicant accomplished on the organization’s official stationery/letterhead, signed by the applicant’s immediate supervisor (if applicant is employed)
7. Fully Accomplished Permit to Study (if applicant is employed)
8. Photo ID file (soft copy, printable as 2×2 and 1×1 photos – use white background, must have name and signature as label, business attire)
9. Front and back of applicant’s government-issued ID in **one** file
10. Selfie photo of applicant holding the same government-issued ID in item 7
11. Two (2) Recommendation Forms filled out on behalf of the applicant by former professors or current work supervisors (to be sent by recommenders to TMC MTM admissions email)
12. Proof of payment of Application Fee (may be settled after Entrance Exam)
